• Maintain the confidentiality of your login credentials.
• Notify us immediately of any unauthorized access to your account.
• Use the portal only for its intended purposes related to your healthcare.
• Origins Integrative Health is not responsible for any unauthorized access to your patient portal account resulting from your failure to maintain the security of your login credentials.

7. Appointment Requests & Forms

Submitting an appointment request form or contact form through our website does not guarantee an appointment or establish a patient-provider relationship. A patient-provider relationship is only established upon completion of a formal intake process and an in-person or telehealth visit with one of our licensed providers.

Submission of a form on our website does not constitute a medical consultation and any information submitted through our website forms should not be used in place of professional medical advice.

8. Telehealth Services

Origins Integrative Health offers telehealth services for eligible patients. Telehealth services are subject to the following conditions:

• Telehealth services are available to patients located in Oregon at the time of their appointment.
• Not all services are available via telehealth — please contact our office to confirm availability.
• Telehealth visits are subject to the same appointment and cancellation policies as in-person visits.
• Insurance coverage for telehealth varies by plan — please verify your coverage before scheduling.
• Please note that certain insurance providers including Providence do not currently cover telehealth services

9. SMS Consent and Text Messaging

Subject to your express opt-in consent, Origins Integrative Health LLC will periodically send SMS text messages to the mobile number you provide. SMS communications are available only to individuals 18 years of age or older. By opting in, you confirm that you are at least 18 years old and that you are the subscriber of the mobile number provided, or that you have authority to opt that number in.

Two categories of messages may be sent, and each requires its own consent:

• Transactional and patient care messages: appointment reminders and confirmations, scheduling changes, care coordination, billing and payment notifications, and replies to your inquiries.
• Marketing messages: clinic news, health education, and promotional offers. You receive these only if you separately consent to marketing messages.

How to opt in: check the SMS consent box on a form on our website, give your number and consent verbally to our staff (which we document in your record), or text our business number at 503-314-2176.

Message frequency varies. Message and data rates may apply. Consent is not a condition of receiving care or of purchasing any service.

• Opt-out instructions: reply STOP to any message at any time. You may also email info@originspdx.com or call 503-314-2176.
• Help: reply HELP for assistance, or call 503-314-2176.
• Opting out of SMS communications will not affect your ability to receive information from us through other channels, such as email or phone.
• Mobile information sharing: mobile information, including text messaging originator opt-in data and consent, will not be shared with third parties or affiliates for marketing or promotional purposes. Such information is used solely to deliver the SMS communications you have consented to.
• Mobile carriers are not liable for delayed or undelivered messages.

Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges for healthcare items and services if you are uninsured or choose not to use your insurance. You can request a Good Faith Estimate before scheduling a service or at any time.
