Perineural Injection Therapy in Portland

Gentle injections that calm irritated nerves.

Perineural injection therapy (PIT) at Origins Integrative Health helps Portland patients with pain that comes from irritated or sensitized nerves. The injections are shallow, quick and use a gentle dextrose solution. Care is offered by Dr. Heather Krebsbach and Dr. Carly Martin at our clinic on SE Ankeny St.

What perineural injection therapy is

Many long-lasting pain problems involve small nerves just under the skin that stay on high alert. When those nerves are irritated, they can keep sending pain signals even after the original injury has settled.

PIT places a low-concentration dextrose (sugar) solution just under the skin, along the path of those nerves. It helps calm them down so they can return to normal function. Because the injections are shallow, many patients find them easier than they expected.

What PIT can help with

People come to us for PIT when pain has lingered or keeps coming back. It’s often part of care for:

  • Knee, shoulder and elbow pain
  • Back and neck pain
  • Wrist pain, including carpal tunnel symptoms
  • Foot and heel pain
  • Nerve-related pain after an injury or surgery
  • Headaches with a nerve component

Research supports PIT for nerve pain. A randomized, placebo-controlled trial published in Mayo Clinic Proceedings found that perineural dextrose injections reduced pain and improved function in people with carpal tunnel syndrome.

Your PIT visit

Your first visit starts with a conversation and an exam to find which nerves are involved in your pain. We’ll explain what we find and what we suggest before anything begins.

The injections themselves are quick and shallow, and many people notice the area feels calmer soon after. Most people go straight back to their usual day, and you’ll leave with clear aftercare guidance.

Who performs PIT at Origins

PIT at Origins is offered by Dr. Heather Krebsbach, ND, LAc and Dr. Carly Martin, ND. Both take time to understand where your pain comes from and build a plan around your body and your activity. Dr. Heather also offers prolotherapy, which can be paired with PIT to support the deeper joint tissue.

PIT is not covered by insurance. Call us for pricing.

PIT is one part of our musculoskeletal care and our headache care. Many patients pair it with prolotherapy, shockwave therapy or acupuncture. To learn more, read Dr. Krebsbach’s article Prolotherapy and PIT for joint pain. When you’re ready, contact us to book a visit.

asked questions

Does perineural injection therapy hurt?

Most people find it very manageable. The needle is small and the injections stay just under the skin, so they feel like quick, light pinches.

How is PIT different from a cortisone shot?

PIT uses a gentle dextrose solution rather than a steroid. It works by calming irritated nerves so they can return to normal function.

Can PIT be combined with other treatments?

Yes. Many patients pair PIT with prolotherapy, shockwave therapy or acupuncture. We'll talk through what makes sense for your pain at your visit.

Is PIT covered by insurance?

It's not covered by insurance. We go over your plan at your first visit, and you can call us for pricing.
